Workers Comp:

If you are injured on the job you MUST report your injury to your immediate supervisor or school nurse. Employees who are not in a specific school building with a nurse available will report injuries to their immediate supervisor or Karen Dowd at Central Office 973-764-4241. If you travel within district and need immediate assistance, report to the closest school nurses office.  

You will be asked to complete a First Report of Injury form in detail.  Click HERE to view the First Report of Injury form.  The School Nurse will call Qual-Lynx at 1-800-425-3222.  If you are present at the time of the call, you must be prepared to give all detailed information to the Qual-Lynx representative.  The school nurse will give you instructions on how to proceed.

Use of a medical provider approved by Qual-Lynx is required if treatment beyond the school nurse is needed.  A Qual-Lynx nurse case manager will direct your care accordingly.  Please do not seek treatment unless directed by a nurse case manager. 

If you are injured after your building office staff has left for the day and it is an emergent situation, you should seek emergency medical treatment and follow up with your building office staff or nurse as soon as possible. 

In the event you are injured after school office hours and do not require urgent treatment, please complete a First Report of Injury form and notify your supervisor as soon as possible.

When seeking medical treatment for a work-related injury, the following information must be given to the Urgent Care or Emergency Department in order to prevent needlessly being billed by the provider:
